Bartlesville Women's Network Historymaker of the Year Award Nomination Form

This award will be presented during our annual March event celebrating National Women's History Month.

The History-Maker of the Year Award Criteria is:

• May be nominated by a Women's Network member of by any Bartlesville resident.
• Nominee must not be a member of current WN Board.
• Your name,
  address,
  and telephone #.
• Nominees should be advocates/pioneers devoted to improving the human condition in any arena of human life. Their contributions should also have the effect of advancing the status of women either by example or direct action. The basis for nomination can be a single achievement or the cumulative activities of a lifetime.
